Dear Student-Athletes and Parents,
Please carefully read all of the information on the following pages before completing the necessary forms on the Magnus Health website. Incoming freshmen and transfers, you will need to check your FalconMail for your Magnus Health username and password. You will be asked to sign a statement saying that you have read and understand our policies, so please be sure to do so. Parents must sign this Procedures document on the Magnus portal if your child is covered under your health insurance plan. Parents must complete and sign all forms for any athlete under 18 years of age.
All forms must be submitted in order to be eligible for participation in DSC athletics. If forms are not completed in their entirety by the due date, you will be responsible for getting your own physical at Halifax Hospital at your cost. All physicals must be conducted by Daytona State College’s team physicians.
All student athletes must provide documentation from a physician regarding current and/or previous history of medical conditions, including concussion, cardiac incidents, surgeries, and all other injuries. Written clearance from a medical doctor to participate in athletics, written on his/her letterhead,must be provided for any injury incurred within the last year for which a doctor was consulted and/or which prevented you from participating in your sport. If you are currently under the care of a physician or undergoing physical therapy or rehabilitation for an injury, you must submit all paperwork associated with that injury in order to be eligible to participate in any athletic activity at Daytona State College.
Copy of Current Insurance Card
You must upload a copy of the front and back of all health insurance cards to the designated location on the Magnus Health website. Please include general medical, dental, and vision plans, if applicable. If the student-athlete’s insurance changes during the school year a new copy must be submitted, and the new information provided by logging in to the Magnus website.
Sickle Cell Testing
All student-athletes must show proof of sickle cell trait status or be tested at Daytona State College on the day of the physical. Instructions for uploading documents are provided on the Magnus Health website.
Athletic Physical Examination
All student athletes must have an athletic physical examination conducted by DSC Medical Staff beforethey participate in any College-related athletic activity. Examinations are conducted at Halifax Sports Medicine Clinic or in the DSC Athletic Training Clinic on assigned days and times. Students should check with their head coach as to the date and time of their exam. Failure to show for an assigned physical will result in the student having to pay for an outside physical arranged by the Office of Sports Medicine with our team physician.
Students may find themselves placed on a Provisional status after their physical exam. This means that some additional information, testing or monitoring of the student-athlete is required in order to be eligible for team participation. Students may play or practice with the team as long as they comply with the restrictions or recommendations of the Sports Medicine Staff. Failure to meet deadlines or cooperate with the Sports Medicine Staff while on aprovisional status will result in the student-athlete being ineligible to participate in ANY College athletic activity. Students who have not been cleared by the DSC team physician may NOT play or practice with the team until specific tests or necessary information is obtained.
